Understanding Your Child’s Behavior & Strengths

This screening tool helps you observe and understand different areas of your child’s behavior and their strengths in everyday living. It’s designed to give you insights into how your child typically responds to various situations.

Please answer each question based on how often your child has shown the described behavior or characteristic during the past 6 months. Your honest observations are valuable.

1. Everyday Skills (Adaptive Difficulties)

(How often your child struggles with daily living, social interaction, or communication)

A. Has difficulty with daily routines (e.g., dressing, hygiene, chores).

B. Struggles to make or keep friends.

C. Has difficulty understanding social cues or rules.

D. Has trouble expressing needs or ideas clearly (verbally or nonverbally).

E. Shows poor problem-solving skills in everyday situations.

2. Worry & Nervousness (Anxiety)

(How often your child shows signs of worry, fear, or nervousness)

A. Worries a lot about various things.

B. Is unusually shy or sensitive.

C. Gets nervous in new situations or with unfamiliar people.

D. Shows physical signs of nervousness (e.g., fidgeting, trembling, stomach aches when worried).

3. Sadness & Low Mood (Depression)

(How often your child shows signs of sadness, hopelessness, or loss of interest)

A. Seems sad, unhappy, or tearful most of the time.

B. Has lost interest in activities he/she used to enjoy.

C. Complains of being tired or having low energy, even after rest.

D. Expresses feelings of worthlessness or hopelessness.

4. Physical Complaints (Somatization)

(How often your child expresses physical complaints without clear medical cause)

A. Complains of stomach aches or nausea (not due to illness).

B. Complains of headaches or other body aches (not due to injury/illness).

5. High Energy & Restlessness (Hyperactivity)

(How often your child is overly active, restless, or impulsive)

A. Is constantly in motion, can’t sit still.

B. Has difficulty waiting his/her turn or interrupting.

C. Talks excessively or loudly.

D. Is restless or fidgety.

6. Anger & Hostility (Aggression)

(How often your child shows angry, hostile, or physically aggressive behaviors)

A. Argues a lot with adults.

B. Has temper tantrums or explosive outbursts.

C. Physically attacks or fights with others.

D. Bullies or is mean to other children.

E. Threatens or intimidates others.

7. Rule-Breaking Behaviors (Conduct Problems)

(How often your child shows rule-breaking, defiant, or antisocial behaviors)

A. Lies or cheats.

B. Steals things.

C. Breaks rules at home or school.

D. Is defiant or refuses to obey adults.

8. Focus & Attention (Attention Problems)

(How often your child has difficulty focusing, is easily distracted, or is impulsive)

A. Has difficulty concentrating on tasks or activities.

B. Is easily distracted by irrelevant sounds or sights.

C. Makes careless mistakes or misses details.

D. Fails to finish assignments or chores.

9. Unusual Behaviors (Atypicality)

(How often your child shows unusual or odd behaviors, thoughts, or perceptions)

A. Behaves in strange or peculiar ways.

B. Repeats words or phrases over and over.

C. Has unusual fears or obsessions.
